Registration for New Students

For in-person registration, parents have to visit our school and fill out the following forms at the office.

For online registration, parents have to download and fill out the forms manually. The scanned registration form and at least one ID card indicated below have to be e-mailed to “office@nebulaacademy.ca” or dropped off at the school.

Before sending/filling out the forms, please check the registration checklist:

2. Proof of the student’s age and legal name AND Alberta Health Card.

(Documents accepted are: Canada birth certificate, PR Cards or permanent landed immigrant/residence documentation, passport, Canadian citizenship papers, adoption certificate, or student authorization visa  (office staff can make copies for you)

3. An intake appointment may be required for new students, school administration will contact parents accordingly.
